Buccal massage is intra-oral work: a practitioner works the mid face and jaw from inside the mouth, through the cheek wall, reaching muscle and fascia that cannot be addressed from the surface. Buccal massage works with tension and fluid. It does not reduce muscle mass the way an injectable does, and it does not alter bone or fat pad position. If what we are seeing is structural, we will say so.
